Please double-check that you have filled in the form correctly. The name will be shown on your delegate badge and conference certificate. 请仔细确认填写是否正确，该姓名将会显示在你的代表牌及参会证书上。

Guardian Name 监护人姓名
If you are at least 18 years old and mentally sound, it means you have full legal capacity for civil conduct and can independently engage in any civil activities. In the guardian information section, please write: “I have full legal capacity for civil conduct.” 如果你已年满十八周岁且精神健康，则代表你已具有完全民事行为能力，能够通过自己的独立行为进行任何民事活动，请在监护人信息中填写“本人已具有完全民事行为能力”。
Guardian ID/Passport Number 监护人身份证/护照号码
Guardian identification information will only be used to provide to the public security authorities when the minor delegate checks into the conference hotel. 监护人证件信息仅用于未成年代表入住会议酒店时向公安机关提供。
Guardian Phone Number/Emergency Contact Number 监护人电话/紧急联系人电话
If you are 18 years old or older, please provide your emergency contact number. 如果你已年满十八周岁，请填写紧急联系人电话。
School/Institutation Name 学校/机构全称
